Our Services

At Juntos Family Support, we help adults with developmental disabilities stay safe, supported, and connected while living at home with the people who care for them most. We are a family-run service, so we understand the responsibility and love that comes with supporting a family member. Our support is bilingual, personal, and built around your family’s routines, comfort, and goals. Juntos Family Support is vendorized through the Regional Center of the East Bay, which allows us to work closely with case managers to provide approved family support services for families in our community.

Initial In-Home Assessment

We review the case manager’s intake form, meet with the family, clarify support needs, and identify goals. After the visit, we create a written report recommending hours and services.

Benefits & Resources Guidance

We help families understand programs they may qualify for, including IHSS, Medi-Cal, Social Security, and local community services. We guide you through forms and next steps.

Family Support Coordination

We assist with coordinating support services, scheduling appointments, and connecting families with community programs to ensure safety, independence, and inclusion.

Ongoing Support Check-Ins

We check in regularly to update goals, monitor progress, and adjust services as your family’s needs change over time.
