Family Support Assessment

Our Family Support Consultation is a one-on-one meeting where we sit with your family, review your needs, and help you understand what services, resources, and supports are available. We guide you step by step and make sure you feel informed and supported.

Referral From Your Case Manager

Your case manager is the first step. They send us a referral requesting an in-home assessment. Once we receive it, we reach out to your family to introduce ourselves and find a day that works best for you.

In-Home Family Assessment

We visit your home and take time to understand your family’s routines, strengths, and challenges. We review the intake form, ask follow-up questions, and make sure every voice in the home is heard. Our goal is to learn what support will truly help your loved one and your family.

Written Report & Support Plan

After the visit, we prepare a clear, easy-to-understand report. It includes recommended hours, support needs, and how Juntos Family Support will assist your family in the home and community. We send this to your case manager and keep you updated every step of the way.
